Incident Reporting Procedure

Our commitment

Nimbin Youth is committed to the safety and wellbeing of all young people, staff, volunteers and visitors. We take incidents seriously and respond in a timely, respectful and transparent way to reduce harm and improve safety.

What is an incident

An incident is any event that causes, or could cause, harm, distress or risk. This may include:

  • injury or medical emergencies
  • safety hazards or near misses
  • challenging or unsafe behaviour
  • breaches of the Code of Conduct
  • child protection or wellbeing concerns
  • property damage or security issues

Immediate response

If an incident occurs:

  • ensure everyone’s immediate safety
  • provide first aid or emergency support if required
  • contact emergency services (000) where necessary
  • seek support from another staff member or coordinator

Reporting an incident

All incidents must be reported as soon as practicable to the Nimbin Youth Coordinator or delegated responsible person.

Incidents should be documented:

  • as soon as possible after the event
  • factually and without judgement
  • using clear, accurate information

Child safety incidents

If an incident involves:

  • harm or risk of harm to a young person, or
  • a disclosure or suspicion of abuse

It must be managed in line with the Child Protection Policy and mandatory reporting requirements under NSW law. This may include reporting to external authorities.

Recording and confidentiality

Incident reports are:

  • stored securely
  • treated as confidential
  • shared only on a need-to-know basis

Information will be used to support safety, accountability and learning.

Follow-up and review

Nimbin Youth will:

  • review incidents to understand contributing factors
  • take steps to reduce future risk
  • provide support to those affected
  • update practices or procedures where needed

No blame approach

Nimbin Youth promotes a learning-focused approach. Reporting incidents helps keep everyone safe and does not result in negative consequences when raised in good faith.
